We’d like to thank everyone who has taken part in our survey of patients about the care and support they received after leaving hospital.

The survey has now closed and we are analysing the responses. The publication of our report will follow.

Over the last two years we have heard from some people that the process of leaving hospital and arranging care after time in hospital, has not gone as smoothly as they would have liked.

We have been working with North Tyneside Council and the NHS to explore this in more depth, to find out what works well and what could be better.

We invited feedback from:

  • Patients in hospital who were about to leave and needed further support
  • People who had recently been discharged from hospital and needed ongoing care or support
  • Family members, carers and people who had given those above their support

Investigating care and support when you leave hospital is an ongoing project for Healthwatch North Tyneside. Along with the survey, we have also begun talking to people in person in several local settings, and plan to expand on this with future engagement work.
